The Department of Social Development and Labor – SEDEST will be responsible for issuing the cards as part of the Autism Awareness Month actions.
To request the card, it is necessary to present the following documents: ID, CPF, updated proof of residence in Juazeiro do Norte, medical report specifying the CID, blood test and two 3×4 photos.
If the request is made by the person responsible, caregiver or curator, the necessary documents are: ID, CPF, updated proof of residence in Juazeiro do Norte.
If the request is made by the caregiver, a signed and stamped statement from the Guardianship Council must be presented, specifying the service requested. For requests made by the curator, it is necessary to present the legal document of the curatorship certificate.
