A 38-year-old white woman, diagnosed with relapsing-remitting multiple sclerosis at age 28, seeks care for further evaluation. The patient reports a 10-year history of recurrent paresthesia, in addition to significant depression, anxiety and fatigue. She has no history of smoking, use of alcoholic beverages or illicit drugs, nor does she have a consistent family history, uses concomitant medications or has comorbidities. His current medications are dimethyl fumarate 240 mg twice daily and fluoxetine 40 mg daily. Over the last year, the patient reports worsening weakness, decreased coordination and irregular spasms in the right hemiface of recent onset; these signs and symptoms had a significant impact on her quality of life. Furthermore, the patient observes an increase in the frequency of relapses of relapsing-remitting multiple sclerosis during this period.
